Job Announcement- Program Manager

Health Projects Center, an independent local non-profit health

organization established in 1980, is hiring a Program Manager for the Del Mar Caregiver

Resource Center (DMCRC) and California Community Transitions Program (CCT). The

DMCRC provides education and supportive services to caregivers and families of adults

living with cognitive impairments. The CCT assists people living in skilled nursing

facilities to transition to community based living.


Manage and deliver supportive services including: in-home assessments, care planning, support groups, conferences, workshops, individual education, referrals and respite care.
Supervise, train and evaluate team of two full-time Family Consultants and Transition Coordinator (plus interns, volunteers, part-time staff and consultants).
 Ensure that DMCRC and CCT goals for service delivery are met.
 Coordinate care planning meetings.
 Develop new services to meet the emerging needs of caregivers.
 Support and motivate team to achieve program objectives.
 Develop and improve systems and procedures.
 Ensure compliance with all funder requirements.
 Provide timely and accurate reports for directors and funders.
 Represent Health Projects Center to partners and the public.
 Serve as a member of the Health Projects Center Leadership Team.


 Master’s degree in social work (LCSW preferred), psychology, nursing, counseling, rehabilitation, gerontology or sociology required.
Demonstrated skills effectively developing, managing and implementing programs.
Significant experience providing direct services to elderly and/or cognitively impaired individuals and/or their families.
Supervisory experience.
Demonstrated leadership skills.
Excellent written and verbal communication skills.
Willingness to travel within Monterey, Santa Cruz, and San Benito Counties regularly.
Fluency in both English and Spanish is preferred.
Current CA driver’s license and reliable, insured vehicle required.


This is a full time position based in our Santa Cruz office. Competitive salary based on
